Alcidion expands NHS partnership with $7.5M Miya Flow contract
Alcidion Group Limited has signed an expanded contract with Northern Care Alliance NHS Foundation Trust (NCA), deepening an existing relationship rather than marking a first-time client win. The agreement carries a Total Contract Value (TCV) of $7.5M over an initial three-year term, with options to extend for a further two years. In-year revenue expected to be recognised is approximately $4.5M, a figure that includes a six-month renewal of the ExtraMed subscription running concurrently while Miya Flow is being implemented across the Trust.

What the contract covers: Miya Flow across four NCA hospitals
NCA is one of north-west England’s largest healthcare Trusts, providing hospital and community healthcare services across Greater Manchester. Under the expanded agreement, Miya Flow will be deployed at Salford Royal Hospital, replacing the existing ExtraMed patient flow solution, and to additional sites within NCA.
The four hospitals covered by the deployment are:
- Salford Royal Hospital
- Royal Oldham Hospital
- Fairfield General Hospital
- Rochdale Infirmary
ExtraMed became part of the Alcidion group following its acquisition in 2021, meaning this contract represents a product upgrade within an established installed base rather than a displacement of a competing vendor. The contract also includes options to expand to additional modules, providing further upside within the existing commercial structure.
What is Miya Flow and why does it matter for hospitals?
Patient flow software helps hospitals manage the movement of patients through their facilities more efficiently, from admission through to discharge. In large NHS Trusts operating across multiple hospital sites, coordinating this process manually creates significant friction, including beds sitting unavailable while paperwork is processed, patients placed in wards outside their clinical specialty (known as outliers), and clinical staff carrying excessive administrative workloads.
Miya Flow is designed to address these specific challenges. The platform centralises bed management and patient tracking information, giving clinical and operational teams a clearer, real-time picture of capacity and demand across a hospital network.
The ExtraMed product it replaces was an earlier-generation solution. Miya Flow is positioned as a next-generation upgrade, offering broader functionality and cross-site coordination capabilities rather than a like-for-like swap. For an NHS Trust such as NCA, which manages four hospitals across Greater Manchester, a unified digital platform capable of operating at that scale carries practical advantages over maintaining separate or legacy systems at individual sites.

Why this contract matters for ALC investors

NCA was already an ExtraMed client, and rather than seeking an alternative vendor at renewal, the Trust chose to standardise on Miya Flow and broaden its deployment across the full hospital network. From a revenue visibility standpoint, the approximately $4.5M in-year recognition provides a near-term financial contribution, while the $7.5M TCV underpins forward revenue across the initial three-year term. The two-year extension option and module expansion rights offer additional contracted upside without requiring new client acquisition.
Alcidion operates across three established markets, Australia, New Zealand, and the United Kingdom, with the UK NHS representing a meaningful portion of its commercial pipeline. The NCA expansion adds to that presence without the cost or risk profile associated with entering new accounts.
The NCA expansion sits alongside Alcidion’s third UK EPR platform win at University Hospitals Sussex, a seven-year Miya Precision deal worth approximately $35M TCV that cemented the company’s position as a recurring reference point across NHS procurement decisions.
The announcement does not include any update to full-year financial guidance, and no forward earnings projections should be inferred from this contract alone.
The FY26 guidance reconfirmation, issued alongside completion of the Kyra acquisition in late June 2026, placed full-year revenue above $50M and EBITDA above $5.0M, with management citing long-term contracted revenue as the basis for that confidence.
