INOVIQ’s CAR-exosomes kill up to 90% of ovarian cancer cells in new lab study

INOVIQ Limited (ASX:IIQ) reported positive in vitro proof-of-concept data on 25 June 2026, demonstrating that its CAR-exosome platform killed up to 90% of ovarian cancer cells in laboratory testing.

The study showed CA125-targeting CAR-exosomes killed more than 90% of OVCAR-3 and Caov-3 cells within 48 hours, while EGFR-targeting CAR-exosomes eliminated over 80% in the same models. In the aggressive, treatment-resistant SK-OV-3 line, both approaches achieved approximately 50% tumour cell killing.

The result extends INOVIQ’s “off-the-shelf, cell-free therapeutic” platform across a third hard-to-treat tumour type, following earlier proof-of-concept data in triple negative breast cancer (TNBC) and non-small cell lung cancer (NSCLC).

Inside the ovarian cancer results

The study evaluated INOVIQ’s CAR-exosomes, derived from engineered immune cells, across three industry-standard ovarian cancer cell lines. OVCAR-3 and Caov-3 are widely used lines derived from high-grade serous ovarian adenocarcinoma, while SK-OV-3 is recognised for aggressive behaviour and multidrug resistance.

Across all three lines, the CAR-exosomes outperformed unmodified extracellular vesicles. All effects were statistically significant, with experiments performed in triplicate and significance reported at p<0.0001.

Cell Line Cancer Profile CA125 CAR-exosome Killing EGFR CAR-exosome Killing
OVCAR-3 High-grade serous ovarian adenocarcinoma >90% >80%
Caov-3 High-grade serous ovarian adenocarcinoma >90% >80%
SK-OV-3 Aggressive, multidrug-resistant ~50% ~50%

Key method highlights from the study included:

  • Exposure of cell lines to CAR-exosomes at 2.5 million total particles per well

  • A 48-hour exposure period prior to assessment

  • Cell survival measured via xCELLigence

  • Tumour cell death confirmed by Annexin/propidium iodide staining and flow cytometry

  • Statistical analysis performed using ordinary one-way ANOVA

The company noted the results suggest multi-targeting CAR-exosomes may address antigen heterogeneity and remain feasible in highly aggressive cancers.

CAR-exosome In Vitro Efficacy Across Ovarian Cancer Cell Lines

What CAR-exosomes are and why investors should care

Exosomes are nanoscale extracellular vesicles secreted by cells that play key roles in intercellular communication. CAR-exosomes are a next-generation cell-free therapy derived from modified immune cells, inheriting the tumour-targeting and cytotoxic capabilities of their parent cells.

The investment angle centres on the platform’s design. Unlike living cell therapies, CAR-exosomes are described as an “off-the-shelf, cell-free therapeutic” approach that may offer advantages in manufacturing, safety and tumour penetration, particularly in solid tumours where current cell therapies face significant challenges.

The addressable markets are large and underserved. According to INOVIQ, more than 320,000 women are diagnosed with ovarian cancer worldwide each year, while TNBC represents 10–20% of the over 2.3 million breast cancer cases globally. Both indications are characterised by aggressive disease, high recurrence and limited treatment options.

CSO Commentary

“These positive ovarian cancer results further strengthen the evidence supporting the therapeutic potential of our CAR-exosome platform across multiple solid tumours. Importantly, we have now demonstrated tumour-killing activity in TNBC, NSCLC and ovarian cancer models using different tumour-targeting approaches. Our focus is now on advancing a differentiated next-generation CAR-exosome product with enhanced anti-tumour activity and scalable manufacturing capabilities to support progression toward first-in-human studies targeted for 2028,” said Dr Rebecca Lim, Chief Scientific Officer.

An integrated approach to ovarian cancer

The therapeutic data connects to INOVIQ’s broader strategy in ovarian cancer. The company is pairing its CAR-exosome treatment programme with its EXO-OC blood test for ovarian cancer detection, framing the work as an integrated detection-plus-treatment thesis rather than a standalone therapeutic.

CEO Commentary

“Our CAR-exosome platform continues to generate promising preclinical proof-of-concept data across multiple tumour types, supporting its potential as an off-the-shelf, cell-free therapeutic platform for treatment of hard-to-treat solid tumours. These new CAR-exosome results, together with our EXO-OC™ blood test for the detection of ovarian cancer, reinforce INOVIQ’s integrated approach to combining earlier ovarian cancer detection with more effective treatment to improve patient outcomes and help save women’s lives,” said Dr Leearne Hinch, Chief Executive Officer.

Next steps and what to watch

INOVIQ is advancing its CAR-exosome programme through preclinical and manufacturing development activities to support future clinical trials. The company outlined several workstreams currently underway:

  1. GMP-compliant cell sourcing

  2. Selection of manufacturing partners (CDMOs)

  3. Optimisation of next-generation dual-action CAR-exosomes, designed to target tumour cells and overcome the hostile tumour microenvironment

For investors tracking near-term catalysts, new in vitro tumour-killing data from dual-action CAR-exosomes in TNBC is expected in Q4 CY2026. Looking further ahead, first-in-human studies are targeted for 2028.

These time-bound milestones provide investors with both near-term and longer-term datapoints to monitor as the platform moves from preclinical validation toward the clinic. All data reported to date remains preclinical and in vitro, with efficacy in humans yet to be established.

Frequently Asked Questions

What are CAR-exosomes and how do they differ from CAR-T cell therapy?

CAR-exosomes are nanoscale, cell-free vesicles derived from engineered immune cells that inherit the tumour-targeting and cancer-killing capabilities of their parent cells. Unlike living CAR-T cell therapies, they are designed as an off-the-shelf product that may offer advantages in manufacturing, safety, and penetration of solid tumours.

What did INOVIQ's ovarian cancer CAR-exosome study show?

INOVIQ's in vitro study showed CA125-targeting CAR-exosomes killed more than 90% of OVCAR-3 and Caov-3 ovarian cancer cells within 48 hours, while EGFR-targeting CAR-exosomes eliminated over 80% in the same lines; in the aggressive, multidrug-resistant SK-OV-3 line, both approaches achieved approximately 50% tumour cell killing.

When does INOVIQ plan to start first-in-human clinical trials for its CAR-exosome platform?

INOVIQ has targeted first-in-human studies for 2028, with the company currently advancing GMP-compliant cell sourcing, CDMO partner selection, and next-generation dual-action CAR-exosome optimisation as preparatory steps.

What is the next near-term data catalyst for INOVIQ's CAR-exosome programme?

INOVIQ expects to release new in vitro tumour-killing data from dual-action CAR-exosomes in triple negative breast cancer in Q4 CY2026, providing investors with a concrete near-term milestone to monitor.

What is INOVIQ's EXO-OC test and how does it relate to the CAR-exosome programme?

EXO-OC is INOVIQ's blood test designed for the detection of ovarian cancer, and the company is pairing it with its CAR-exosome therapeutic programme to build an integrated detection-plus-treatment approach aimed at improving patient outcomes in ovarian cancer.
